Movie: THE MAIN EVENT, rated TV-G
What do you need to Bring? Sleeping Bag / Blanket and Pillow (attach names please)
For Ages: children 4 years old through 12 years old.
Deadline to Register: Thursday, June 23rd at 7:30pm
Legal stuff:
By Registering and Paying I am agreeing to the following: I hereby submit application for my child to attend the Saltamacchio's Tang Soo Do Parent's Night Out Event. I assume full responsibility for all my child's actions while in attendance at said event. I understand that my child will not be allowed to leave the school premises from 6:30 pm on with anyone other than myself (their guardian) or a person I have designated to do so. Master Ray must receive in writing (text or email acceptable 706-473-1556) the name of any authorized person sent to pick up my child/children. I give my consent that any and all pictures/video's taken at event may be used on Facebook or any other media advertising our Karate School.
